前列腺癌分期结果为医疗护理团队提供关于癌症大小及生长速度的信息。分期有助于医疗护理团队制定治疗方案。

分期信息

医疗护理团队使用以下信息来确定前列腺癌分期：

  • 前列腺内的癌变范围。 如果癌症仅累及部分前列腺，则分期较低。如果癌症累及前列腺的更多部分，则分期可能较高。
  • 癌细胞是否生长到前列腺以外。 如果癌细胞穿透前列腺壁，则被视为 3 期。癌细胞可能穿过前列腺，扩散至直肠、膀胱或其他邻近部位。
  • 癌细胞是否扩散至淋巴结。 前列腺癌可能通过淋巴系统扩散至周围淋巴结。如果发生这种情况，则被视为 4 期。
  • 癌症是否扩散至身体其他部位。 前列腺癌可能扩散至骨骼、肝、肺及身体其他部位。如果发生这种情况，则被视为 4 期。
  • 血液中的 PSA 水平。 PSA 水平升高通常意味着分期更高。在分期评估中，低 PSA 水平是指低于 10 纳克/毫升（ng/mL）。中等水平是指介于 10 至 20 ng/mL 之间。高 PSA 水平是指高于 20 ng/mL。
  • 分级分组。 分级分组以格利森评分为基础。分级范围为 1 到 5 级，数字越低表示癌细胞生长可能越缓慢。数字越高表示癌细胞侵袭性越强、生长速度可能越快。分级分组的数字越高，分期越高。

前列腺癌分期使用数字和字母来描述癌症的严重程度及其生长速度。数字为 1 到 4。数字越小通常表示癌灶越小、生长速度越慢。癌灶越大或侵袭性越强，分期数字越高。某些分期还包含字母，如 2A、2B 和 2C。这些字母将数字分期进一步细分为更小的类别。

1 期

前列腺癌 1 期意味着癌灶较小且癌细胞仅存在于前列腺。癌症累及范围不到前列腺一侧的一半。PSA 水平低，分级分组 1 级。

癌症 1 期治疗通常包括主动监测，因为癌症病灶较小且生长缓慢。不过，一些较年轻且愿意为了治愈癌症而接受某些副作用的患者可能会选择接受治疗。

2 期

前列腺癌 2 期意味着癌细胞仅存在于前列腺，但与 1 期相比，累及的腺体范围可能更大。2 期包括低等和中等 PSA 水平，分级分组包括 1 至 4 级。

  • 2A 期。 前列腺癌 2A 期为分级分组 1 级。在此分期，癌灶可能较小，仅累及前列腺一侧，但 PSA 处于中等水平。或者，癌灶也可能较大，同时累及前列腺两侧，但 PSA 处于低水平。
  • 2B 期。 前列腺癌 2B 期为分级分组 2 级，且 PSA 处于低或中等水平。此分期涵盖尚未扩散至前列腺以外的任何大小的肿瘤。
  • 2C 期。 前列腺癌 2C 期为分级分组 3 级或 4 级，且 PSA 处于低或中等水平。此分期涵盖尚未扩散至前列腺以外的任何大小的肿瘤。

2 期治疗可采用主动监测，尤其是在 PSA 水平和分级分组均较低的情况下。如果选择治疗，治疗方案通常包括手术切除前列腺（前列腺切除术）和放射疗法。放疗方案包括外照射疗法和近距离放射治疗。

3 期

前列腺癌 3 期意味着癌症比 2 期更严重。癌症可能已经扩散到前列腺之外。这一分期包括高 PSA 水平和分级分组数值高的癌症。

  • 3A 期。 前列腺癌 3A 期患者的 PSA 处于高水平。这一分期包括分级分组 1 级至 4 级。此分期涵盖尚未扩散到前列腺以外的任何大小的肿瘤。

3 期治疗通常包括治疗癌症本身，除非患者有其他更为紧急的健康状况。3 期癌症具有很高的扩散风险，因为癌症已经生长到前列腺以外，或表现出快速生长的迹象。治疗方案可能包括手术或放射治疗联合激素治疗。

4 期

前列腺癌 4 期意味着癌症已扩散至前列腺以外。这种情况也称为转移性前列腺癌。4 期癌症可能包括任何 PSA 水平和任何分级分组。

  • 4A 期。 4A 期前列腺癌已扩散至周围淋巴结。
  • 4B 期。 前列腺癌 4B 期已扩散至身体其他部位，如骨骼、肝和肺。

4 期治疗通常从激素治疗开始。如果癌症已扩散至淋巴结，或许仍然可以选择前列腺手术和放疗。如果癌症扩散至身体其他部位，通常采用能够在全身任何部位杀灭前列腺癌细胞的药物。

关于分期的常见问题

以下是前列腺癌患者关于分期的一些常见问题：

  • 早期意味着什么？ 早期前列腺癌意味着癌变仅在前列腺内。通常是 1 期或 2 期癌症。
  • 有前列腺癌 5 期吗？ 没有前列腺癌 5 期，分期从 1 至 4 期。但是有分级分组 5 级。前列腺癌分级分组的组别包括 1 到 5 级。在前列腺癌中，分级分组 5 级表示癌细胞形态与健康细胞差异显著，并且可能快速生长。所有分级分组 5 级前列腺癌均属于 3B 期，无论其体积如何。
