An immunoglobulin kappa and lambda free light chain test is a blood test that measures small proteins in your blood called free light chains. These proteins may be a sign of a plasma cell disorder. There are two types of free light chains: kappa and lambda.
The kappa and lambda free light chain test shows:
- The level of kappa free light chains.
- The level of lambda free light chains.
- The balance between the two, called the kappa-lambda ratio.
You also may hear this test called:
- Serum free light chain test.
- Serum free light chain assay.
- Immunoglobulin free light chain test.
Kappa and lambda light chains are found in antibodies, also known as immunoglobulins. The two types of light chains perform the same basic function. Antibodies are proteins that help your immune system recognize and fight germs. Along with heavy chains, light chains help form the part of an antibody that recognizes and attaches to a specific target, such as part of a germ.
When light chains are not attached to heavy chains, they are called free light chains.
Typically, the body makes more kappa free light chains than lambda free light chains. Lambda free light chains tend to pair up, which makes them longer. Their larger size causes the kidneys to remove them more slowly from the blood.
One type of free light chain is not worse than the other. What matters for a kappa and lambda free light chain blood test is:
- The pattern of both levels.
- The ratio between them.
- Kidney function.
- Other test results.

Your care team may order a kappa and lambda free light chain test to look for or track certain blood proteins. The test can help care teams diagnose a condition, such as a plasma cell disorder, or see how well a treatment is working.
Kappa and lambda free light chains are sometimes associated with specific blood conditions. These conditions happen when one group of plasma cells makes too much of the same protein.

When it comes to diagnosing a specific condition, a kappa and lambda free light chain test is just one part of a larger puzzle.
A result outside the reference range does not by itself mean you have kidney disease. It also does not mean you have an autoimmune disease or a plasma cell disorder, such as myeloma. A slightly elevated or high free light chain level is an example of a result outside the reference range. Kidney function and infections, such as the flu, also can change free light chain levels.
Many care teams use a free light chain test with other blood and urine tests. For instance, you may have a blood test called serum protein electrophoresis. It separates proteins in the blood and can show a monoclonal protein, also called an M protein. M proteins and free light chains can provide different information about plasma cells. Looking at both can help diagnose certain conditions.

A healthcare professional usually takes the blood sample from a vein in the arm. The main risks are soreness and bruising where the needle goes in. Most people don't have a strong reaction to having blood drawn.

If you are having only a kappa and lambda free light chain test, you usually do not need to do anything special to prepare. You can eat and drink as usual. But the test is often ordered with other tests. Follow the instructions you receive.
You can return to your usual activities right after the procedure.

During the procedure
A member of your healthcare team takes a blood sample from a vein in your arm, usually near the bend of the elbow. The needle is attached to a small tube that collects the blood. You may feel a quick pain when the needle goes in. Tell the person drawing blood if you feel dizzy or unwell. Pressure is placed on the site after the needle is removed to help stop bleeding.
After the procedure
After a blood draw, you can return to your usual activities right away. You may have short-term soreness or bruising where the needle was placed. Most people do not have a strong reaction to having blood drawn.
The sample goes to a lab for testing. The time needed for results can vary by lab. Ask your care team when you can expect the results and how they will be shared.

Your results usually show three numbers:
- Your kappa free light chain level.
- Your lambda free light chain level.
- Your kappa-lambda ratio.
Your care team looks at all three numbers together because different patterns can have different causes. One number alone usually cannot show what is causing a rise or fall in light chain levels.
The ratio compares the levels of each free light chain. It can show whether the body is making much more of one type of free light chain than the other. For instance, both levels may rise when the kidneys don't clear light chains well or when the immune system is active. If one level is much higher than the other, the pattern may point to a plasma cell disorder.
Other testing is needed to confirm what is causing a specific free light chain pattern.
What a high kappa level may mean
A high kappa level doesn't by itself show what is causing the rise. Kappa levels can rise when:
- The kidneys clear light chains more slowly.
- The immune system is active, which can happen with an infection or inflammation.
- One group of plasma cells makes too much kappa.
The kappa-lambda ratio and other test results help your care team understand what may be causing the high kappa level.
What a high lambda level may mean
A high lambda level can happen for the same broad reasons as a high kappa level. The kidneys may be clearing light chains more slowly. The immune system may be active. Or one group of plasma cells may be making too much lambda. A low kappa-lambda ratio may point to extra lambda production.
When both kappa and lambda are high
Sometimes both kappa and lambda levels are high, but the ratio stays within the reference range. This pattern often happens when the kidneys remove light chains more slowly or when the immune system is active. Your care team may review kidney tests and other protein tests to understand the result.
Infections, such as coronavirus disease 2019 (COVID-19), can activate the immune system and raise free light chain levels too.
When one level is much higher than the other
There is no single kappa-lambda ratio that is concerning for everyone. A result outside the reference range does not by itself mean you have cancer or even a blood condition. The pattern may be caused by a kidney condition.
Sometimes one group of plasma cells is making too much of the same light chain. This is called a monoclonal gammopathy. Some monoclonal gammopathies, such as MGUS, may not cause symptoms and may only need follow-up. Others, such as multiple myeloma or light chain amyloidosis, may need treatment.
The kappa-lambda ratio can be affected by either extra kappa production or extra lambda production. One pattern is not necessarily more serious than the other. Factors your care team looks at includes:
- How far the result is from the reference range.
- The amount of the light chain being made.
- Kidney function.
- Other test findings.
If you have an unusual ratio, you may need imaging or bone marrow testing as part of a broader evaluation.
What happens next
Your care team may compare the result with other tests, such as protein electrophoresis, immunofixation, urine testing and earlier free light chain results. The test also can be repeated over time to monitor some plasma cell disorders and to help show a response to treatment. How often you have the test depends on your condition and your care team's recommendations.
