Stages

Breast cancer staging describes how much cancer is in the body and whether it has spread. Knowing the stage helps your healthcare team understand the cancer and plan treatment.

Breast cancer stages range from 0 to 4. In general, a lower number means the cancer is smaller or has spread less. Stage 4 means the cancer has spread to a distant part of the body.

It can take time to determine the stage. Your healthcare team may not know the final stage until after surgery, when more information about the cancer is available.

Not everyone needs imaging tests to find out whether breast cancer has spread. Depending on your situation, tests may include:

Types of staging

Your healthcare team may describe breast cancer stage in different ways.

  • Anatomic stage. This is based on the size of the cancer and whether it has spread to nearby lymph nodes or other parts of the body.
  • Prognostic stage. This combines the anatomic stage with other information about the cancer, including tumor grade and ER, PR and HER2 status.

Breast cancer stages

Breast cancer stages are:

  • Stage 0. Stage 0 breast cancer is called ductal carcinoma in situ (DCIS). In DCIS, abnormal cells are found inside a breast duct. They haven't spread into the surrounding breast tissue. Some DCIS has hormone receptors.
  • Stage 1. The cancer is small and may only be in the breast. In some cases, a small amount of cancer is found in nearby lymph nodes. The cancer hasn't spread to distant parts of the body.
  • Stage 2. The cancer may be larger than stage 1 cancer or may have spread to nearby lymph nodes. It hasn't spread to distant parts of the body.
  • Stage 3. The cancer has spread further within the breast, to nearby lymph nodes or to nearby tissues. It hasn't spread to distant parts of the body.
  • Stage 4. The cancer has spread to a distant part of the body. This is called metastatic breast cancer.

Stage isn't the same as tumor grade. Stage describes how much cancer is in the body and whether it has spread. Grade describes how different the cancer cells look from healthy cells and can help show how quickly the cancer may grow.
