A team-based approach to care

In Community Internal Medicine, your primary care professional leads a team that works together to give you the best possible care. This team figures out what you need, organizes your care and makes sure you get the right help at the right time. They also help connect you with specialists when needed.

Your care team includes doctors, nurse practitioners, physician assistants, pharmacists, registered nurses, licensed practical nurses, medical assistants, clinical assistants, and appointment line and customer service staff.

To help your team take care of you, let them know if you get medical care outside of Mayo Clinic. For example, if you go to an emergency room or are admitted to a hospital that isn't part of Mayo Clinic, tell your care team. You also can ask other doctors to send your medical records to your primary care professional at Mayo Clinic.

Test results

You usually can see your test results online as soon as they're ready. Your primary care professional will explain what the results mean and tell you if you need any treatments or more tests. You'll get this information within seven business days after your last test. You may get the information in different ways:

• A secure electronic message through the Mayo Clinic Patient Portal.
• A voice message through MedVoice, a confidential automated notification system.
• A phone call from a member of your care team.
