Mayo Clinic's approach to treating Peyronie disease

June 26, 2025

Peyronie disease is a condition characterized by the development of fibrous scar tissue in the penis, which can lead to penile deformity, pain and sexual dysfunction. Although it may not always cause overly bothersome symptoms, this disease can lead to significant psychological distress for many patients, affecting both self-esteem and relationships in addition to causing physical burdens.

Unfortunately, there is a significant amount of misinformation surrounding Peyronie disease, often leaving patients pursuing treatments that provide little to no benefit. This misinformation not only prolongs their journeys to effective care but also may result in unnecessary physical and emotional challenges. Medical professionals should be aware of the prevalent myths and misconceptions surrounding this condition to better guide their patients toward the correct treatment options.

"At Mayo Clinic, we take an individualized approach to evaluating and treating patients with Peyronie disease. We take the time to perform a thorough evaluation and discuss all the available and evidence-based treatment options, but we tailor the treatment plan based on shared decision-making between our team and the patient," says Matt J. Ziegelmann, M.D., a fellowship-trained urologist at Mayo Clinic in Rochester, Minnesota, with extensive clinical and research expertise in Peyronie disease.

"This is not a one-size-fits-all condition, and we do not take a one-size-fits-all approach. Some patients benefit from noninvasive options, while others are better suited for injection or even surgical therapies up-front. Experience matters, and patients should know they are receiving care from a practice with some of the highest volumes in the country," says Dr. Ziegelmann. "This is true for our procedural and surgical practice, but it's also important for patients to know we have the expertise to address their psychological burdens as well through counseling and partnership with mental health experts," he adds.

Harnessing the benefits of virtual health

Dr. Ziegelmann and his team are also acutely aware of the time and financial commitments that come with pursuing treatment at an institution away from home. They work hard to coordinate with patients, offering opportunities for follow-up visits and even postoperative care through a telemedicine platform. They also relish the opportunity to partner with referring physicians whenever possible.

"The explosion in telehealth services has really improved my ability to care for patients throughout the country and even the world. For example, we can accomplish nearly all our postoperative care virtually for patients undergoing penile grafting and plication procedures, including planned follow-up visits and counseling," says Dr. Ziegelmann.

Bringing the operating room to the clinic

In addition to having some of the highest surgical volumes for Peyronie disease, Dr. Ziegelmann has also pushed the boundaries in terms of where and how these surgeries take place. He routinely performs penile plication — a procedure to straighten penile curvature in patients with Peyronie disease — in the office using only local anesthesia. This allows patients to avoid the potential harms of anesthesia, limit the time spent in the hospital, and even drive themselves to and from the procedure.
