Mayo Clinic physicians are committed to collaborating with referring physicians. Telephone consultations and referrals may be arranged by contacting the Referring Physician Service. The office is available 24 hours a day, seven days a week.

This answering service will record the information. Appointment secretaries will call the referring physician (or physician's office) back between 8 a.m. and 5 p.m. Central time.
Appointment secretaries are available to record information and arrange appointments directly in Oral & Maxillofacial Surgery. To facilitate referrals, please complete and fax a Patient referral form for each patient.
