Accepted plans

Coverage limitations apply to all Medicaid programs listed below. A pre-service deposit of the estimated charges will be required for unauthorized services. Please contact your insurance company directly about benefit coverage and pre-authorization requirements.

• Arizona. Transplant services for patients with Arizona Health Care Cost Containment System (AHCCCS). Authorization required by AHCCCS prior to your appointment.
• Florida. Transplant services for patients with Florida Traditional Medicaid when applicable. Does not participate in Medicaid Managed Care Plans at this time.
• Rochester, Minnesota. Participates with state Medicaid programs of Iowa, Minnesota, North Dakota, South Dakota and Wisconsin. Managed Care Medicaid Plans require a referral from a PCP and approved authorization.

Out-of-state Medicaid is not accepted

Mayo Clinic does not accept and cannot bill Medicaid from non-participating Medicaid patients from states other than those listed in the accepted plan section above. If you're covered by Medical Assistance or a Medical Assistance health plan sponsored by a state that isn't listed above, you are considered self-pay and a pre-service deposit of the estimated charges will be required.
