1Some exclusions apply. Global emergency medical evacuation services are only available to members under the age of 84, for medical emergencies occurring 150 miles outside of their home areas. Fees associated with the evacuation are covered if arranged through the healthcare professional and deemed medically necessary. Medical evacuation policy covers up to two evacuations a year for each member.

2Some exclusions apply. Lab draws are only available to patients within a 30-mile radius of Mayo Clinic in Florida. Mayo Clinic 365 Elite members receive up to five complimentary in-home lab draws, with additional draws available on a fee-for-service basis.

Membership fees

What is the fee for the program?

Membership in Mayo Clinic 365 requires an annual fee. This is in addition to costs for specific medical services. Pricing is based on the level of care you select. Please call 904-956-0006 to speak with a representative who can help you find the care plan that is right for you.

When is the fee due?

The initial fee is due with your membership agreement. The annual fee is billed on the anniversary of your membership.

Does Mayo Clinic bill my fee on a monthly or quarterly basis?

No. The fee must be paid in a lump sum on an annual basis to maintain your membership.

Does my insurance or health plan pay for the membership fee?

No. If you have a healthcare reimbursement account, it's possible the fee could be reimbursed by that account. Please check with the adviser who manages it.

Is the annual membership fee tax deductible?

Please check with your tax adviser for tax advice.

Do I have to pay the membership fee even if I don't use my doctor's services during that year?

Yes. The annual membership fee covers the cost of access to your doctor even if you don't use it.

Access and enrollment

How does Mayo Clinic provide more immediate access?

Mayo Clinic 365 doctors limit the number of people accepted into their practice. This ensures more time for your personalized, continuous care. As a member, you'll also have unrestricted 24/7/365 access to your concierge medicine doctor via cellphone or the user-friendly Mayo Clinic Patient Portal.

How can I enroll in Mayo Clinic 365?

Members are accepted on a first-come, first-served basis. To be accepted in the program, you must complete a membership agreement and pay your annual fee.

Does Mayo Clinic accept Medicare reimbursement?

Yes. We bill Medicare for covered services. Medicare doesn't reimburse for the membership fee.

Does Mayo Clinic participate in other insurance programs and health plans?

Yes. A list of our participating insurance plans and health plans is available upon request. The membership fee is in addition to charges for specific medical services.

Doctors

What are the credentials of these doctors?

All Mayo Clinic 365 doctors are board certified in internal medicine and are employed by Mayo Clinic. They were selected for their excellent reputations, commitment to service and demonstrated ability to build in-depth relationships with patients.

Is my doctor available to me while I'm traveling or out of town on an extended basis, including at my homes in other locations?

Yes. Your doctor is available to you by phone or portal message no matter where you are. When your doctor isn't available due to personal travel, one of their colleagues is available.

Medical services

What about the fees for tests, specialists and hospitalization?

Those charges are billed to your insurance, either private or Medicare, and are reimbursed according to your benefits. You are responsible for any fees not covered by your insurance.

You may select from several health and wellness services aimed at helping you develop a sharper mind, stronger body and an overall improved sense of well-being.

Optional services

Multicancer early detection test. Mayo Clinic is proud to offer a new cutting-edge early cancer detection test, which has demonstrated the ability to detect multiple types of cancers through a single blood draw. Most of these cancers can't be detected through current screening models. The results of this test are shared with you in your Mayo Clinic patient portal or reviewed at your Mayo Clinic 365 appointment.

If a cancer signal is detected, your Mayo Clinic 365 doctor may talk with you about potential additional testing for accurate diagnosis. If a cancer signal is not detected, your doctor likely won't recommend follow-up tests. This test does not replace any of Mayo Clinic's standardized testing or guidelines recommended for cancer screening.

Predictive genomics services. The Mayo Clinic Department of Clinical Genomics offers comprehensive, cutting-edge diagnostic testing using next-generation sequencing techniques. It offers an array of genetic testing services to help evaluate possible genetic diagnoses. A genetic counselor reviews and evaluates your family medical history and discusses genetic testing options.

Pharmacist consult and testing. A consult with a pharmacist with testing to provide education about medicine and to review all your medicines and supplements is called a pharmacogenomics consult. Testing includes the OneOme RightMed 27-gene panel. This is a once-in-a-lifetime test that helps determine how you metabolize medicines. This testing helps eliminate trial-and-error prescribing. This type of test may be covered by your insurance.

Nutrition consult. A consult with a registered clinical dietician is individualized to your needs. The goals are to help you achieve and maintain a healthy weight and maximize physical and mental health through proper nutrition.

Weight management consult. The number of options available to help you manage your weight can be overwhelming. At Mayo Clinic, our team of weight-management experts can help determine what option is best for you.

We offer solutions that have been shown through research to support success. These include:

  • Lifestyle changes, such as diet, exercise and stress management.
  • Medicines.
  • Nonsurgical options.
  • Surgical options.

Stress management consult. This consult helps you understand the neuroscience of stress. The care team member shares with you effective tools to decrease stress and increase resiliency by enhancing engagement and emotional intelligence. This service may be covered by insurance.

Cosmetic consult. A cosmetic consult explores elective cosmetic skin options such as Botox, fillers and optimal care regimens. It's available at Mayo Clinic Cosmetic Center in Jacksonville, Florida. This service isn't covered by insurance.

Massage therapy. Indulge yourself in a relaxing, full-body massage while at Mayo Clinic. Treatments are self-pay with cash, check or credit card.

Running assessment. A running assessment is a comprehensive biomechanical assessment. It takes an integrated approach to improving your running performance and preventing injuries. After the assessment, you receive a personalized plan to help you achieve your goals and prevent injury.

Travel consult. A travel consult includes a vaccination review and other travel-based medical advice.
