Cochlear implants offer significant auditory and quality-of-life improvement for adults who derive limited benefit from hearing aids. Otologists and neurotologists at Mayo Clinic in Florida frequently perform cochlear implantation in adults with moderate to severe bilateral sensorineural hearing loss, asymmetric sensorineural hearing loss and single-sided deafness.
Cochlear implantation has evolved in recent years, from expanded candidacy criteria to advances in electrode design and surgical techniques. The technology's impact is life-changing, as Mallory J. Raymond, M.D., a neurotologist at Mayo Clinic in Jacksonville, Florida, observes in her practice.
"About 85% of patients will experience a significant improvement in their clarity or word understanding in their implant ear, with average gains of 40% to 50%," Dr. Raymond says.
Evaluating patients for cochlear implant candidacy
Expanded candidacy criteria now allow implantation in patients with greater degrees of residual hearing, thanks to data highlighting the benefit. Physicians still must make nuanced, evidence-based recommendations, as criteria vary by insurance provider.
A practical screening approach incorporates two components of standard audiometric testing, commonly referred to as the 60/60 guideline:
- Pure-tone thresholds requiring ≥ 60 decibels to detect sound.
- Unaided word recognition scores ≤ 60%.
Hearing tests often reveal borderline candidates with word recognition scores near 60%. A formal cochlear implant evaluation can help identify definitive candidates. In Mayo Clinic Otolaryngology (ENT)/Head and Neck Surgery, patients undergo specialized audiologic testing with the use of hearing aids, guided by a cochlear implant audiologist. The multidisciplinary evaluation also includes surgical consultation, cochlear implant counseling and education, questionnaires, and imaging, typically completed all in one day.
"We are still understanding and working on ways to perfectly match someone's individual anatomy with the size of the electrode and insertion techniques, both before and during surgery."
While background noise during audiologic testing may help patients qualify, clinical decision-making is not always straightforward. "Our team regularly discusses whether to perform a cochlear implant to ensure that we present a balanced set of recommendations to the patient," Dr. Raymond says.
Subjective indicators of progressive sensorineural hearing loss
Mayo Clinic otologists and neurotologists also consider patient-reported changes in hearing aid benefit as an important assessment component. Common patient-reported concerns include difficulty:
- Hearing compared with a prior baseline.
- Hearing in environments with background noise.
- Understanding words in conversation.
- Using the telephone.
Special considerations in single-sided deafness
Compared with patients with bilateral hearing loss, patients with single-sided deafness often experience tinnitus and difficulty localizing sound.
Cochlear implantation in this population offers benefits beyond speech clarity, including tinnitus suppression and improved spatial hearing. For many patients, tinnitus relief is a primary motivating factor for pursuing implantation.
A personalized approach to electrode selection
Identifying the optimal electrode for each patient requires careful consideration. "We are still understanding and working on ways to perfectly match someone's individual anatomy with the size of the electrode and insertion techniques, both before and during surgery," Dr. Raymond says.
Surgeons choose between straight and precurved electrodes:
- Straight electrodes are very thin and sit along the outer cochlear wall.
- Precurved electrodes sit closer to neural structures at the center of the cochlea, potentially improving outcomes but with an increased risk of insertion-related complications.
Clinical factors also guide the decision-making process. For example, stiffer, stylet-based electrodes may be used in patients with cochlear fibrosis or ossification, while thinner, sheath-based electrodes are often selected when residual hearing preservation is a priority.
"I feel that I have a higher chance of preserving residual hearing with thinner electrodes," Dr. Raymond explains. "If I do not, or if it diminishes with time, and I've chosen a precurved electrode, then I still have the benefit of having that curvature close to the neural structures."
Surgical techniques to optimize outcomes
Alongside electrode selection, surgical technique continues to evolve with a focus on minimizing trauma and preserving hearing. Surgeons now prefer the round window approach over cochleostomy to access the scala tympani, also known as the tympanic duct, through the cochlea's natural opening.
Robot-assisted electrode insertion
Mayo Clinic ear surgeons often use robotic cochlear implant insertion technology — currently limited to straight electrodes — to place the electrode at a slow, controlled pace.
"Studies have shown that this type of insertion leads to the least insertional trauma and lowest amount of force," Dr. Raymond says. "I use this tool to try to preserve hearing in patients who may have better residual hearing or asymmetric hearing in the opposite ear."
Intraoperative and postoperative strategies to support outcomes
Mayo Clinic physicians employ multiple methods to confirm electrode integrity and placement, including:
- Electrically evoked compound action potential measurements to assess auditory nerve response to stimulation.
- Intraoperative transimpedance matrix testing to evaluate electrode position, including assessing for tip fold-over.
- Intraoperative X-ray to detect tip fold-over, especially for precurved sheath-based electrodes.
- Steroid administration during surgery or steroid taken orally after surgery to reduce inflammation.
Patients typically undergo device activation at 2 to 4 weeks postoperatively, followed by regular audiology visits. Most patients demonstrate the largest measure of functional improvement within the first three months after activation but continue to improve incrementally over the first year. In many cases, follow-up frequency with an audiologist over the first year can be reduced.
Quality-of-life assessments before and after implantation
Throughout patient evaluation, implantation and follow-up, quality-of-life assessments are central to achieving patient goals and understanding outcomes.
The Cochlear Implant Quality of Life questionnaire provides a validated framework that helps physicians tailor treatment recommendations preoperatively and supports patient counseling by helping set realistic, real-world expectations for cochlear implant outcomes.
The questionnaire also allows healthcare professionals to measure impact postoperatively, including listening effort, communication ability and social engagement.
Emerging research published in Otology & Neurotology has shown that cochlear implantation may positively affect the experience of communication partners. In an ongoing prospective, survey-based study initiated by Dr. Raymond, Mayo Clinic neurotologists are working to better understand the quality-of-life impact of cochlear implantation on patients and their communication partners.
