John Sullivan, D.O., FAAFP, is a Senior Associate Consultant in the Department of Family Medicine at Mayo Clinic in Jacksonville, Florida, where he provides care for individuals and families throughout Jacksonville and surrounding areas. He also serves as an Assistant Professor of Family Medicine at Mayo Clinic College of Medicine and Science.
