Augustine Chavez, M.D., is a Family Medicine physician with interest in population health and health equity. He completed his residency at University of New Mexico School of Medicine in Albuquerque, New Mexico. He is the Associate Chair of Practice in the Department of Family Medicine where his clinical work focuses on comprehensive, multi-disciplinary team-based care. He is Co-director of the Student Community Clinic — a longitudinal clinical experience in care for the underserved, and Population Health Domain Leader in the Science of Health Care Delivery at Mayo Clinic Alix School of Medicine.

He is Board-certified by the American Board of Family Medicine and is Assistant Professor and Consultant at Mayo Clinic Arizona, Department of Family Medicine.
