A Study to Evaluate the Effectiveness of Diaphragmatic Breathing on Reflux Symptoms in Outpatients Jacksonville, Fla. The purpose of this study is to establish if abdominal breathing exercises can reduce symptoms associated with reflux. Effect of Cholecystokinin (CCK) on Gastroduodenal Activity in Humans Rochester, Minn. The purpose of this research study to compare the effects a hormone cholecystokinin (CCK) in the bloodstream in healthy people and in patients with rapid gastric emptying. CCK regulates (controls) the rate at which food empties from the stomach into the small intestine. We want to learn how CCK affects the activity of the stomach and small intestines in patients with rapid gastric emptying. We are also trying to learn if genes regulate the effects of CCK. A Study to Assess Intestinal Barrier Function and Permeability in Patients with Functional Dyspepsia Jacksonville, Fla. The purpose of this study is to investigate, using several novel techniques, potential pathophysiologic abnormalities that could lead to the development of symptoms of functional dyspepsia. A Study Of Intratumoral Injection Of Autologous Dendritic Cells After High-Dose Conformal External Beam Radiotherapy In Patients With Unresectable Liver Cancer Rochester, Minn. The primary purpose of the pilot study is to evaluate the safety and tolerability of an autologous dendritic cells (DC) vaccine delivered by intra-tumoral injection in patients with primary liver cancer treated with high-dose conformal external beam radiotherapy (EBRT). The primary purpose of the phase II study is to estimate the progression-free survival rate at 2 years post-registration to see if treatment is efficacious compared to historical data. Mayo Clinic Inflammatory Bowel Disease Repository Rochester, Minn., Jacksonville, Fla., Scottsdale/Phoenix, Ariz. The purpose of this research study is to collect clinical information and human samples from patients with IBD. The data collected will be used to create an organized, accessible resource of human samples for biological research in IBD. Utility Of Random Biopsies In Patients With Inflammatory Bowel Disease Jacksonville, Fla., Rochester, Minn. The proposed study is a multicenter parallel group clinical trial that will include 821 evaluable patients per group who will be randomly assigned to either high definition white light colonoscopy (HDWLC) with targeted biopsies plus 2 random biopsies in 4 segments to assess for inflammation (limited biopsy strategy) or HDWLC with targeted biopsies plus 4 biopsies every 10 cm throughout the colon, at a minimum in all segments of the colon known to have been affected by IBD at any time, regardless of the extent of disease (random biopsy strategy). Participants will be followed until total proctocolectomy or the end of the study period to determine whether the two methods of surveillance colonoscopy are associated with detection of dysplasia or sessile serrated adenoma at follow-up colonoscopy. Follow-up via chart review may continue for up to 15 years from enrollment. Adverse effects related to Heplisav-B vaccination in patients with Inflammatory Bowel Disease Jacksonville, Fla. In this study, we will examine the potential side effects experienced after receiving the new hepatitis B vaccine (Heplisav-B) in patients with ulcerative colitis or Crohn's disease. This new vaccine has been found to be more effective than the existing vaccine Engerix in people without ulcerative colitis or Crohn's disease. It is now the standard of practice vaccine that we use in the Mayo gastroenterology and liver practice. The most common side effects include pain, redness, swelling, fatigue, fever and headache. We would also like to determine if there was any worsening of the patient's underlying inflammatory bowel disease. Part 1 Given these findings and recommendations, members of the Gastroenterology and Infectious Disease department feel that the Heplisav-B vaccine should be the preferred vaccine for patients with IBD and chronic liver disease. In this study, we aim to collect data on patients who receive this vaccine on whether they experience any adverse effects or any worsening of their bowel or liver disease. We also aim to collect data regarding the effectiveness of vaccination by measuring post-vaccination antibody titers at 1 month after vaccination. The goal of this is to confirm that patients have achieved immunity after vaccination. This is consistent with best-practice guidelines and CDC recommendations in immunosuppressed persons. Part 2: According to the 2022 Center for CDC guidelines for Hepatitis B vaccination in immunocompromised adults, persons found to have antibody titer concentration <10 mIU/ml (indicating non-immunity) after completion of a primary vaccine series should be revaccinated [6]. Accordingly, in the case of non-immunity after the standard 2-dose Heplisav-B series, we aim to give an additional dose of Heplisav-B and re-measure antibody titers at 1 month in order to evaluate effectiveness. This is consistent with best-practice and CDC guidelines in immunosuppressed patients that have not responded to the primary vaccination series. To this point, the CDC guidelines for Hepatitis B vaccination in immunocompromised adults state that a larger vaccine dose or additional vaccine doses may be necessary in this patient population [6]. Immunity to Hepatitis B through vaccination in patients with inflammatory bowel disease and chronic liver disease is crucial due to high risk of morbidity. Part 3: As a continuation of the above, we aim to perform immunological testing to observing T cell response to Heplisav-B vaccination. The goal is to measure if there are any differences in T cell response depending on certain immunosuppressive medications that the patient is taking for their IBD.
