Hazel Patel, D.O.

Hazel Patel, D.O., is a family medicine physician who provides comprehensive primary care for individuals and families at the Mayo Clinic St. Augustine Primary Care Clinic. Dr. Patel's clinical interests include preventive care, chronic disease management, and women's health. She is dedicated to partnering with patients to promote wellness throughout all stages of life and to support long-term health goals through education and collaboration.

Dr. Patel completed her Doctor of Osteopathic Medicine degree at Lake Erie College of Osteopathic Medicine in Erie, Pennsylvania. She is an active participant in research and quality improvement projects focused on enhancing well-child visits and improving outcomes for people living with chronic diseases such as type 2 diabetes and chronic kidney disease. She holds certifications in Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), Advanced Life Support in Obstetrics (ALSO), and the Neonatal Resuscitation Program (NRP). She is a member of the American Academy of Family Physicians and the American Osteopathic Association.

Dr. Patel provides compassionate, evidence-based family medicine care with a focus on prevention and long-term wellness.
