If you are not between 16-80 years of age and not able to travel to Rochester, Minn., 7-8 times during the first 12 weeks of being enrolled in a clinical trial, do not complete this form. You can search www.clinicaltrials.gov to see if another study might work for you.
If your responses suggest that you are a candidate for one of our studies for ulcerative colitis or Crohn's disease, we will call you and ask approximately six additional questions to determine whether or not you are potentially eligible.
If your schedule does not permit participation in a clinical trial at Mayo Clinic, we may be able to provide additional information about similar trials and resources closer to you.
If you are not eligible to participate in a clinical trial at Mayo Clinic, we will inform you by email. If you wish to receive treatment at Mayo Clinic, please respond to the email and we can assist with appointment scheduling.
