Complete the form below to help us determine if you are a potential participant in our inflammatory bowel disease (IBD) clinical trials. All information is kept confidential and never disclosed.
If your responses suggest that you are a candidate for one of our IBD studies, we will call you and ask approximately six additional questions to determine whether or not you are potentially eligible. A final determination is made after you are evaluated by a physician.
If your schedule does not permit participation in a clinical trial at Mayo Clinic, we may be able to provide additional information about IBD trials and resources closer to you.
If you are not eligible to participate in an IBD clinical trial at Mayo Clinic, we will inform you by email. If you wish to receive treatment at Mayo Clinic, please respond to the email and we can assist with appointment scheduling.
After submitting your request, do not leave this form until you see the confirmation message.
Your Mayo Clinic number is a 7, 8 or 9-digit number we assign to you as a new patient prior to your first visit. You can find it near the top of a range of documents, including pre-visit questionnaires, clinical notes, care summaries, and correspondence.
If you do have Mayo paperwork handy, it's often quickest simply to look for it there. Scroll down to see some examples of how the number shows up:
