To refer a patient:

Arizona

Contact the Referring Physician Services at 480-301-6539 or 866-629-6362 (toll-free), 24 hours a day, seven days a week.

Florida

Contact the Referring Physician Services at 904-953-7000 or 800-634-1417 (toll-free), 24 hours a day, seven days a week.

Minnesota

Contact the Ophthalmology Appointment Office at 507-284-2744 from 8 a.m. to 5 p.m. Central time, Monday through Friday. After business hours, contact the Referring Physician Service at 507-538-1515 or 800-533-1564 (toll-free). You can also complete a patient referral form and fax it to 507-538-7355.

To contact the ophthalmologist on call or any other specialty that the referring physician needs, call the Mayo Clinic operator at 507-284-2511.

For urgent or emergency transportation, call Mayo Clinic's emergency communications at 800-237-6822 (toll-free) to request Mayo Clinic ground ambulance, Mayo Clinic air ambulance helicopter or Mayo Clinic air ambulance airplane.

Ophthalmology staff and doctors are happy to work with doctors outside of Mayo Clinic.
