Once registered, suppliers and sales representatives will have access to the following documents and additional information:
- Registration information, documentation requirements, and other disclosures attested by employers include:
- Education and training
- Criminal background checks
- Other training and associated credentials
- Mayo Clinic's Code of Conduct and Integrity Program
- Form to complete annual Conflict of Interest disclosures
- Information related to Patient Confidentiality, HIPAA, and Business Associate Agreements
- Copies of select policies including the Supplier/Sales Representative Access and Privileges Policy; Free Goods Policy; Procurement Policy; Food Policy; and Product Recalls
- Process for scheduling a visit with a Mayo staff member
- Where to report possible violations (i.e., Compliance Office) associated with the new policy
- Completed form attesting that the supplier/sales representative has read and agrees to comply with all applicable Mayo policies related to access and privileges
