Referring a patient

The Department of Radiology is happy to accept referrals and provide consultation to referring physicians. To reach a Mayo Clinic radiologist, call the Referring Physician Service toll-free at 800-533-1564. This number is for use only by referring physicians and their office staff.

Imaging services

The Department of Radiology offers timely, 48-hour imaging second opinion services for medical practitioners.

To request an imaging second opinion:

• Complete Second Opinion Request form and review service rates and billing terms.
• Send the form with a CD/DVD containing the images and report to:
