Aftercare Planning

Although the PRC does not currently provide formal follow-up care, the program coordinates with the patient's home care providers and provides necessary referrals. The PRC's goal is for every patient to continue using the strategies learned at the PRC in his or her home and work environments. A nurse care coordinator will assist the patient with an individual plan for aftercare involving a local primary physician (general internist or family physician), psychiatric/psychological care with a local cognitive-behavioral therapist and review of the tools learned in the program. Occupational therapists will review recommendations for return to work or volunteer work. Physical therapists will help design an independent exercise program to use daily at home.
