Eligibility and qualifications

To be eligible for 2012 Nurse Camp at Mayo Clinic in Arizona, you must be a high school student who has completed your freshman year by May 2012. Enrollment is limited, and participants are selected by a committee of Mayo Clinic health care professionals.

The 2012 camp will take place Monday, July 9 through Friday, July 13. Activities run from 8 a.m. to 4:30 p.m. each day. You must be available to attend the entire week.

How to apply

Applications for the 2012 Nurse Camp are due by May 14, 2012. You are responsible for ensuring that all application materials are complete and postmarked on or before the deadline. Incomplete applications will not be reviewed.

To apply, print and complete these documents:

• The application form; be sure to complete the essay portion at the end.
• A letter of recommendation from a teacher, school nurse or guidance counselor. When requesting your letter of recommendation, you may use this request for a letter of recommendation template.
• The consent to participate form.
• The consent for release form.

The selection committee will choose and notify camp participants by June 11, 2012.
