Referring a Patient

Mayo Clinic nephrologists and hypertension specialists are committed to providing a high level of service to physician colleagues outside of Mayo Clinic. Physicians who refer their patients to Mayo Clinic for speicalized treatment are fully informed of the treatment their patients at Mayo Clinic. In addition, Mayo Clinic specialists are available to help coordinate follow-up and longterm care for patients once they return home to their primary physician.

Nephrology Referrals

To refer a patient for specialized treatment at Mayo Clinic, please call:

    Monday to Friday, 8 a.m. to 5 p.m. Central Time

Hypertension Referrals

To refer a patient for specialized treatment related to hypertension, please call:

Kidney Transplant Referrals

To refer a patient for kidney transplant evaluation, call the Mayo Clinic Central Appointment Office at 507-538-3270.

You may also submit an appointment request directly with the Transplant Department using the following secure online appointment request form. We will reply to online requests at the time you specify on the form (before 4 p.m. Central time daily), within 24 hours or on the next business day.
