The Mayo Clinic Referring Physician Service Center (RPSC) is your primary contact for scheduling appointments and arranging telephone consultations with Mayo Clinic staff members. In addition, the Referring Physicians Service Center will help answer your questions and work to resolve any concerns about Mayo services.

Mayo Clinic's goal is to provide physicians with prompt service and communication of complete results for the patients you refer to us. We will preserve the integrity of your relationship with your patient.

Scheduling Appointments

To schedule patient appointments at Mayo Clinic, please call our Referring Physicians Service at (480) 301-6539 or toll free 1 (866) 629-6362, between 8 a.m. and 5 p.m., Mountain Standard Time, Monday through Friday. If you wish to speak directly to a physician, your call will be directed accordingly. We take care to reinforce your role as your patient's primary care physician and to make clear our role as a consultant. This number is for the exclusive use of referring physicians and their staff.

Hours at Mayo Clinic

Mayo Clinic is open from 6 a.m. to 7 p.m. Mountain Standard Time, Monday through Friday. The clinic is closed Saturdays, Sundays and major holidays.

Information Needed to Schedule Appointments

To schedule an appointment for your patient, Mayo Clinic will need the following patient information:

  • Patient Name
  • Address
  • Phone number
  • Date of birth
  • Mayo Clinic Registration number, if a previous patient
  • Type of insurance (i.e., HMO, Workers' Compensation, Medical Assistance)
  • Nature of the medical problem

As a referring physician, we will need the following information from you:

  • Referring Physician NPI number
  • Services you would like Mayo to provide

Patients with Urgent Needs

Mayo Clinic reserves appointment times for patients with urgent medical needs who require subspecialty care or surgery. If your patient's condition dictates immediate attention, please call the Referring Physicians Service Center at (480) 301-6539 or toll free 1 (866) 629-6362.

Hospitalization

Every effort is made to minimize hospital time. Most diagnosis and treatment can be conducted on an outpatient basis at Mayo Clinic. When patients do require hospitalization, they are admitted to Mayo Clinic Hospital, approximately twelve miles west of our main campus.

Hospital Admissions and Transfers

Should a patient require emergency hospitalization, Mayo Clinic will make arrangements at Mayo Clinic Hospital. To arrange the admission or transfer of one of your patients to Mayo Clinic Hospital, call us seven days a week, 24-hours a day at:

Hospital policy requires you to call Mayo Clinic in advance of the patient's arrival for direct hospital admission.

Insurance Compatibility

Some insurance plans may not be fully compatible with reimbursement for services provided by Mayo Clinic. It is recommended that patients contact their insurance carriers to verify benefit and eligibility for services provided by Mayo Clinic. Patients living within Maricopa County may contact the Mayo Clinic Patient Financial Services department at
