Information For Your Patient
Patient registration
- An individual registration number is given to each patient at Mayo Clinic. This number should be used for all Clinic communications regarding your patient.
- We suggest that you keep a record of your patient's Mayo Clinic registration number for easy reference in future consultation.
- If a patient has previously received a registration number at Mayo Clinic in Minnesota or Florida, that original number will continue to be used.
Insurance
- Our staff will file your patient's primary insurance and will assist patients in filing any supplemental insurance claims.
- Mayo Clinic and Mayo Clinic Hospital are in-network for many insurance plans. Learn more about in-network access.
- Inquiries regarding insurance may be directed to the Patient Accounts Information Line at 480-301-7033 or TDD 480-301-9700.
- Because Mayo Clinic in Arizona does not accept Medicare assignment, payments will be made directly to the patient.
Lodging
- Mayo Clinic is unable to arrange housing for patients. We encourage patients to secure lodging reservations before their anticipated arrival, particularly during the busy winter visitor months.
- A 124-room hotel, Courtyard by Marriott, is located adjacent to the clinic with regular shuttle service to and from the Clinic. Call 602-860-4000 for reservation information.

- Phoenix Sky Harbor International Airport is about 23 miles southwest of Mayo Clinic and is served by major airlines.
