Information For Your Patient

Patient Registration

• An individual registration number is given to each patient at Mayo Clinic. This number should be used for all Clinic communications regarding your patient.
• We suggest that you keep a record of your patient's Mayo Clinic registration number for easy reference in future consultation.
• If a patient has previously received a registration number at Mayo Clinic in Minnesota or Florida, that original number will continue to be used.

Insurance

• Our staff will file your primary insurance and will assist you in filing any supplemental insurance claims.
• Inquiries regarding insurance may be directed to the Patient Accounts Information Line at (480) 301-7033 or TDD (480) 301-9700.
• Because Mayo Clinic in Arizona does not accept Medicare assignment, payments will be made directly to the patient.

Lodging

• Mayo Clinic is unable to arrange housing for patients. A 124-room hotel, Courtyard by Marriott, is located adjacent to the clinic with regular shuttle service to and from the Clinic. Call (602) 860-4000 for reservation information.
• The Scottsdale Chamber of Commerce maintains a current listing of area hotels and a visitors' guide to the area. Their toll-free number is (800) 877-1117.
• We encourage patients to secure lodging reservations before their anticipated arrival, particularly during the busy winter visitor months.

Travel Information

• Phoenix Sky Harbor International Airport is about 23 miles southwest of the Clinic and is served by major airlines.
• SuperShuttle service is available to and from the airport by calling (602) 244-9000.
• Taxi, limousine and car rental services are available in the Scottsdale and Phoenix areas.
