Our kidney transplant program has expanded to include a unique group patients who would otherwise not be able to receive a living kidney transplant by performing the transplant in those sensitized patients with a positive crossmatch against their donor (by flow cytometry) or are ABO blood group incompatible with their donor. Currently, the graft survival of these types of transplants is lower than the standard group of patients. The major reason for the lower survival is the immunologic barrier that must be overcome for a successful transplant, though the patients typically have multiple other factors such as multiple prior kidney transplants and debility from long-standing kidney disease. While the graft survival of these transplants has improved significantly over the past few years, the current SRTR adjustments do not adequately address the variables that will likely continue to decrease graft survival in this group of recipients. The positive cross-match and ABO-incompatible patients are well informed of the increased risk of graft loss, however in many cases, this protocol represents their only remaining option for transplantation.

If these types of transplants are excluded, our center-specific graft survival for both 30 days and 1 year is significantly improved and is within the expected range for our patient population. We provide this clarification to our center-specific survival outcomes as we believe the intent of the center-specific report is to allow for evaluation of outcomes of different programs. Our standard kidney transplant population compares favorably to both the expected and average graft survival rates.
