Founders Group Sponsor - $100,000 commitment
You will be recognized as a Major Benefactor for the Mayo Clinic Health Policy Center (MCHPC). Additional recognition you will receive:

Directors Circle Sponsor - $250,000 commitment

Champion Level Sponsor - $500,000 commitment

• Invitation to participate and recognition at major MCHPC events

Cornerstone Sponsor - $1,000,000 commitment
With a commitment of $1,000,000 or more, Mayo Clinic will be honored to recognize you as one of our valued Principal Benefactors. Additional recognition you will receive:

• Your name will be listed in the Principal Benefactors section in our Hall of Benefactors in Rochester, and featured in appropriate philanthropic media, including our electronic benefactor recognition system located at all three Mayo sites.
• You would be eligible to receive coverage through Preferred Response, a program that protects you while traveling — whether on ground, by air or sea. Mayo Clinic Preferred Response will provide you with remote consultation, translation services, medical evacuation services and a number of preventative services, no matter where you are at your time of need.
• Cornerstone Sponsor commemorative recognition display piece and letter
• Option to have your name published in Mayo Clinic Health Policy Center and Development publications and displays
