Summer Nurse Extern Program
Application Process

Please complete the Mayo Clinic online application and submit the following completed forms by March 1 to Mayo Clinic Hospital – Belinda Curtis, Clinical and Patient Education, 5777 E. Mayo Boulevard, Phoenix AZ 85054. The reference request document requires Adobe Acrobat Reader to view. You must be available for a full ten-week assignment starting in May and ending in August.

• Official or unofficial transcript (obtain from your registrar's office)
