Primary Care Model

Primary care is a method of health care that takes into consideration a whole person rather than individual organs or systems. It involves a team approach that includes physicians, nurse practitioners, physician assistants and nurses. These primary care practitioners offer a range of services, including:

• Health care for existing medical needs
• Preventive care services
• Planning of care for chronic conditions
• Coordination of care with medical and surgical specialists
• Access to the most up-to-date medical information available
• Support for and information about patient self-care

Children who require specialty care can access appointments with over a hundred pediatric and adolescent physician specialists, as well as with physicians in Mayo Clinic's medical and surgical specialty departments. They also have access to Mayo Clinic's comprehensive laboratory testing and imaging (radiology) procedures. If additional consultations are needed, our electronic medical record system allows us to share your care needs with other Mayo colleagues, dramatically speeding the referral process.
