Mayo Clinic is pleased to offer Patient Online Financial Services to Florida residents. The new service is a convenient and secure way to pay your monthly statement, view your itemized statements of account and grant access to your online statements to someone other than yourself.
To use the service, you need a computer with an active internet service provider, a compatible Web browser (Internet Explorer 5.0 works best) and a valid e-mail address. Online payment can be made using a credit/debit card or eCheck. Mayo Clinic accepts Visa, MasterCard, Discover or American Express. eCheck allows you to use your savings or checking account to make an online payment.
All you need to do is complete and return a Patient Internet Communications Agreement form, which requires Adobe Acrobat Reader. Instructions for enrolling as well as more information about the service will be mailed to you once we've received your completed agreement form.
When filling out the form:
Return the first copy of the form to a Registration Desk at Mayo Clinic in Jacksonville or mail it to: Information Retrieval & Support, Mayo Clinic, PO Box 4004, Rochester, MN, 55903-9823. Retain the second copy for your records.
