TRICARE/CHAMPVA

After each visit, Mayo will send you an Itemized Statement of Charges detailing the services and charges for the visit. This document is not a bill. The Monthly Statement of Account is your bill. This statement is a listing of the payments and adjustments made on each visit since your previous monthly statement. Your personal financial responsibility as noted on your TRICARE/CHAMPVA Explanation of Benefits (deductibles, cost share and non-covered charges) should be paid upon receipt of the Monthly Statement of Account.

TRICARE/CHAMPVA as Primary Insurance
Mayo Clinic accepts TRICARE/CHAMPVA assignment. This means that Mayo has agreed to accept the established TRICARE/CHAMPVA fee schedule for your services. Mayo's full fees will be billed to TRICARE/CHAMPVA. As payment is received, your account will be adjusted to the established TRICARE/CHAMPVA fee schedule. You will be responsible to pay for deductibles, cost share and non-covered services. TRICARE/CHAMPVA does not typically pay for your services in full.

TRICARE/CHAMPVA as Secondary Insurance
Insurance claims are submitted to your primary insurance company as services are incurred. Mayo requests that you forward all insurance payments and copies of the Explanation of Benefits from the primary insurance company to:

Your primary insurance company may consider some services to be routine or non-covered and may not provide reimbursement for these services. You are responsible for payment on any services that both your primary insurance company and TRICARE/CHAMPVA determine to be routine or non-covered.

Medicaid/AHCCCS

In Arizona, the Medicaid program is called AHCCCS. Mayo Clinic in Arizona does not contract with AHCCCS except for transplant services. Medicaid/AHCCCS is treated as an HMO requiring authorization prior to delivery of service.
