Surgical Estimates

You may request an estimate from the Precertification Department, which is located on the Concourse level of the Mayo Clinic Building at the Scottsdale campus and on the First floor of the Mayo Clinic Specialty Building at the Phoenix campus. The estimate will provide you some idea as to what your related costs will be. Although we try to estimate as close to the charges as possible, the estimate may vary from the actual charges. The estimate is based on the anticipated procedure that the physician has told us you are having. If the procedure changes and/or the length of the hospitalization is longer, charges will differ.

Included in the estimates are anticipated fees for items such as room and board, operating rooms, anesthesia, surgeons, and when applicable, assistant surgeons. Not included are presurgical consultations and tests. Additional questions should be directed to the Precertification Department or by calling 480-301-8363.

Medicare Estimates

Medicare patients whose physician fees are expected to exceed $500 for procedural or surgical services must obtain an estimate from the Precertification Department.
