Requesting Medical Records and Imaging Mayo Clinic in Arizona

A record of the care patients receive is retained in Health Information Management Services. These records are kept in strict confidence and are not released without the patient's written consent, except as required by law.

Please call Mayo Clinic Health Information Management Services at 480-301-8500 to obtain a copy of your medical information. You can also fax your request to 480-301-7282 using this medical records request form. There is no charge for copies.

Copies of imaging exams (X-rays, ultrasounds, etc.) also are available on a CD or film. Imaging and mammogram films must be requested through Radiology Records at 480-301-8055.

Our usual procedure is to mail your records or imaging exams. You can also pick them up yourself at either Mayo Clinic Hospital (5777 East Mayo Boulevard, Phoenix) or Mayo Clinic (13400 East Shea Boulevard, Scottsdale). Please allow 24 to 48 hours to process your request.
