When arranging an appointment, individuals or referring physicians should provide:

• the patient's name as it is on the passport
• address
• date of birth
• a brief explanation of the current medical problem
• the preferred date of appointment

The International Appointment Office will make the necessary arrangements and fax a confirmation.

All Mayo practices have interpreters to provide interpreting services for international patients upon request. Each site supports a variety of languages, including sign. These services are provided free of charge for the patient's medical care, in both hospital and outpatient settings. If you need an interpreter, please notify the International Appointment Office when arranging your appointment.
