What to Bring

The following list will help you remember what items and information to bring with you to Mayo Clinic.

• The "Registration Information" form (unless you already mailed it in).
• The "Information for Your Physician" form.
• Insurance cards and claims-filing information.
• Medical information (X-rays, records, summaries of past illnesses or hospitalizations) if requested by Mayo.
• List of current medications and dosages.
• Items to help pass the time (reading and writing materials, handwork, etc.).

Also, please be sure to follow any special instructions, such as fasting, that you may have received before your visit.
